CCS Service Facilitator

The CCS program offers a wide array of psychosocial rehabilitation services to clients across the lifespan who have needs for ongoing, high or low-intensity services resulting from mental health or substance use disorders. These are services and supportive activities that assist CCS clients with mental health and/or substance use conditions to achieve their highest possible level of independent functioning, stability, and independence, and to facilitate recovery. Services provided by this position may include:

Service Facilitation
Screening and Assessment
Service Planning
Diagnostic Evaluations
Medication Monitoring
Physical Health Monitoring
Individual Skill Development and Enhancement
Employment-Related Skill Training
Individual and/or Family Psychoeducation
Wellness Management and Recovery/Recovery Support Services
Psychotherapy
Substance Abuse Treatment

The EXPECTATIONS of a HealthyMinds, LLC CCS Service Facilitator are to:

Be recovery-focused
Have knowledge of evidence-based practices for treatment of mental health
Have experience in community-based mental health and substance abuse assessment, case management, treatment planning and/or crisis resolution
Communicate effectively (both orally and in writing)
Work independently and remain organized and efficient while being part of a recovery team
Possess the appropriate professional certification, training, education, experience, and abilities to carry out their prescribed duties
Have proficient computer skills in order to generate reports, complete forms, and document case activity

The QUALIFICATIONS to become a HealthyMinds, LLC CCS Service Facilitator are to:

Have earned a master’s degree in a behavioral related health field (bachelor’s considered)
Have access to a reliable vehicle, maintain a current valid WI driver’s license, and maintain insurance on vehicle
Obtain and maintain CCS certification
